If there is either a new development with your disability or you have been diagnosed with an additional condition, DSS will re-evaluate the current accommodation plan. Updated documentation will likely be required.
To request a re-evaluation, submit an Accommodation Change Request form and upload new documentation through the Accommodate Portal.
If approved for new or updated accommodations, you will receive an updated Letter of Accommodation to share with your Instructors. You will be notified by email when it is ready to download from the Accommodate Portal.
